How to Convert a PDF to a Google Form
So you've got a nicely formatted PDF form or a PDF file with all the background info for a survey, and you're thinking, “it'd be so nice to automatically turn this into a Google Form”.
Good news - we found several ways how you can turn your PDF into a fillable form, including a Google form as well as a more customisable option in case you need more flexibility in terms of design and functionality compared to what Google has to offer. So, let's get started.
Convert PDF to Google Form: Technically Possible, But Not Straightforward
Below we'll present two options for turning an existing pdf document into a Google form. Both are quite easy to implement, but each has their own downsides.
1. AI Form Builder (Extension in Google Workspace Marketplace)

This is probably the first tool that pops up when you search "Convert pdf to google form". It's called AI form builder and works as an add-on in the Google Workspace Marketplace. You can install it here.
Once installed, just head to the Google form builder and the tool will be added as a side panel inside Google Forms interface. For the side panel to be displayed, click on the add-on icon as shown in the image. And that's it on the set-up front!

As you can see, the AI form builder add-on offers various options to create forms, such as Text to Form, File or Image to Form, and even Video or Web Page to Form. Sounds promising… until you realise the “file to form” part, the one that would process your PDF, is behind a paywall. No free trial, no test drive. Not exactly a great start if you're experimenting or on a budget.
You do get one free credit to generate a form from a text prompt. After that, it’s $0.50 per form. That’s totally reasonable if you’re building just one or two surveys a month. But if you're a teacher, researcher, event organiser, or anyone creating forms regularly, that $0.50 starts to add up real fast. Especially when, in the end, you're still working within the limitations of plain old Google Forms.
2. GPT Form Builder inside ChatGPT

Now this one's a little different flow. It’s a custom GPT, called Form Builder Plus for GPT, that lets you create Google Forms directly from ChatGPT. We already wrote about this GPT in a more detailed tutorial. Basically, you can describe the form you want or upload a document inside ChatGPT, and it helps generate the form inside your Google account. This link will take you directly to the plugin.
But again, you still end up with a Google Form. Which is fine, unless you’re looking for more flexibility. Like conditional logic. Or visual customisation. Or complex input types. Well, all the good stuff.
P.S. Since recently, Form Builder Plus for GPT appears to not be operational. We don't know if this is permanent or just a temporary technical problem. But we'll keep an eye on it, and will keep you posted.
